Depends on role. Gyro+rds for any role I need a free hand, bipod + something otherwise.

A flashlight and a bayonet.
Most of the attachments are pointless. The forward grip doesn't reduce recoil at all (screen shake doesn't exist in game for some reason). It used to be my go to attachment, but now I stick with the basic stuff.

Screen shake does exist, but you mostly feel it with gyro.

screen shake exists with both gyro and quickfire adapter. I use the extended barrel to cancel out the adapter's shaking and get the accuracy and the forward for the added on accuracy.

if im going into a fight i try to use shotguns exclusively, to that end bayonets and gyros (though for some reason its sometimes inconvenient to reload with gyros for reasons unknown to me, it keeps saying im not holding the gun im holding), if they close in i can stab the shit out of them and if they break my arm instead of needing both i can use the other one as a spare.
red dot sights to go with that combo if i can help it but they're pretty rare, cargo almost always runs out, would like to see additional rail type attachments at some point because flashlights are too buggy to justify grabbing rail lights alongside my pocket lights.

Depends on what role I am.
I tend to play as a Specialist using either a Sniper Rifle or a SADAR.
If I get a SADAR I will also get a SMG using a Red Dot Sight and a Barrel Charger. The Barrel Charger increases the SMG's damage by 40% while the RDS counters the BC's accuracy penelty.
If I get a Sniper I will use a Bipod and a Barrel Charger, for the same reason as above since the Bipod counters the accuracy penalty.
I will also get two Pistols and attach a flashlight onto each for personal illumination.
Now if by chance I get the B18 Armour I'll use a M41A with a Gyroscopic Stabilizer along with a RDS and either a Charger or a Bayonet, so I can combo the M41A with a Riot Shield.

For Muzzle Attachments I feel that the Bayonet and the Barrel Charger are the top two best ones. The Bayonet allows you to quickly tear down xeno fortifications as well as a reliable melee weapon while the Barrel Charger significantly improves your damage per round which is really damn impressive. The only reason I don't always get a Charger is that they are very rare on the Sulaco.
For Rail Attachments it comes down to a Flashlight or a Red Dot Sight, again the RDS is rare and hard to get so its usually a case of you getting lucky or not needing a RDS. The RDS combos best with other attachments that nerf your accuracy, if your not taking anything that impacts your accuracy then the RDS is not as needed.
For Underbarrel attachments its more situational, two of the three attachments serve to increase your accuracy. The Gyro Stab is very good since it lets you use a gun one handed, which is a great asset. The Bipod pairs best with the Sniper.

Last time I asked a dev he said the Barrel Charger boosts damage by 40%.

its one of those items that people never seem to use as far as i can tell, that and alot of people use their bayonets instead.
that in mind i may start using barrel chargers on my shotguns though, the increase to damage can be the difference against late tier aliens.

I always go for the shotgun as SL/standard/engi, and I find RDS+Extended Barrel+Gyro works best.
If I'm a Spec, I'll go for the sniper with a barrel charger(or suppressor if they're out) and a gyro.
Squad Medic, usually just SMG with red-dot
Tend to have just a normal service pistol for sidearm, maybe with RDS or raillight.
Any Sulaco job (CT, MT, etc,) I just pick up one service pistol and one extra clip, maybe grab an RDS if I can find one.
Command jobs(BO, XO, CO) an SMG w/ RDS + the pistol, with either raillight or RDS.

I only ever use flashlight attachments. I usually end up raiding a couple of lockers so that I can modify the flashlights into mods and attach them to my rifle and my two 44s. Why two you say? Well because I just KNOW that if I don't bring a spare gun I will end up needing it!
