Long time ago, playing as Erika, on commander role, I tried to pick up that sword from the locker.
Since I had itens in both hands, the sword spawned on the floor - in the same positions she was standing in.

I placed the itens on the backpack, and clicked the the sword.
But as I moved my finger on the touchpad, it got registered as a double click or something like that - and that way, Erika picked up the sword and slashed herself on the chest with it.

It got embedded, and she got critically wounded right at round start.

Took some time for the docs to heal her, as no one could access the captain's quarters. The door had to be hacked.
