I have been staff on Hippiestation, although I was deadminned about a year ago. I also co-own a fairly popular Gmod Spacebuild server. (http://bloxgaming.com/). I've been staff on HaloStation, but have since retracted that.
Did you play any of the previous Colonial Marine servers?
Yes, I played back when we had Logistics Officers and a Sulaco that faced North. Good times. (r.i.p Nostromo)
What other servers do you play on?
/vg/station, very rarely paradise/hippie
What are the 3 biggest servers you play on besides CM?
all of the ones mentioned above

I've made a ban appeal, but otherwise no.
Are you currently a staff member elsewhere (not just SS13)? If so, where?
As mentioned above I co-own a garrysmod server, but it's not really a big time consumption and I'd be more focused on this as my partner can take over.

Yes. I Believe it was a while ago for a medical job ban. Other than that , I recieved a 5 day ban for logging out after me and another marine got into a fight about shooting eachother. I accepted the ban as it was bad on my part to leave, but I realized that it was proper escalation and decided to not let it get to me and left. I should have explained to the moderator what happened.
Have you ever been banned for more than 24 hours on ANY server? If so, where and When (roughly)?
I've been banned on Hippie about a year and a half ago, but that was when I first started playing. I've been banned on /vg/ for an incident involving security, although that ban was appealed.
Are you familiar with the chat program Slack (its use is required)?
Yes. We used it on HippieStation.
Common Staff Situations:
Write a description of what you would do in each situation (BELOW) the situation. If you start it on the same line, I'll assume you can't follow directions and auto-decline your application.
1. A player is firing his weapon in the briefing area near the start of the round.
Freeze the player if he is firing into people, question him, if it's griff, I'd inform him of where he can read the rules and why it is wrong, and then a ban based on damage done, apply the punishment relative to the damage. (Most likely nothing above a day ban, and add a note to the users account). If he's just firing around not hitting anyone, ask him why, and possibly add a note to his Ckey. If there is a justified reason, after investigating, punish if the reason isn't justified, and let it play out if justified.(possibly make a MOTHER report about marine firing before briefing even starts).
2. The Sulaco has a breach to space.
Inform the marines of where the breach is through a command report, investigate whether or not it was on purpose. If it was intentional, PM the player a message telling them not to continue, and if their note history was grief-y, apply a ban relative to the amount of grief notes they have. If it wasn't intentional, inform the player to be careful, as breaching is a problem that can ruin rounds, and possibly add a note to the user.
3. The marines want the nuke codes because they claim they are losing too many marines. There are currently 4 aliens and 20 marines in your /who.
Either wait for the AI mother report, or create one informing them of the position they are in. Either way, the nuke is not justified.
4. A Sulaco researcher has made several napalm grenades and hands them out to marines. One of the marines throws the grenade into Logistics. There are no admins or other moderators online.
Bwoink the Marine in question, determine whether or not it was intentional via verbs and the players response. If intentional, apply a 12 hour ban at minimum, scaling up with the amount of previous grief notes they have. If non-intentional, inform the marine to be wary of what they are holding in the hand and the liability he has if marines are injured in it's throwing, add a note detailing the incident. This one if very iffy because determining if it was intentional or not is a cloudy line.
5. A played is starting to talk about the things he did with your mom last night over Ahelp.
Ignore him. (No response necessary). If it continues, mute from Ahelp and add a note to the players Ckey.
6. You notice a marine with a name that doesn't fit our naming rules.
Check the marines notes, If previous warnings, inform the marine that he needs to change his name by the next round. If not, inform the marine in question that his name violates the server rules, and how it breaks them. Consult with another admin if online, and get their opinion about the situation.
6a. The marine is arguing that: They've used the name for several years, noone has told them to change it before, and that they didn't know it was a rule.
Inform the marine that as the game loads in, in big red letters, it says: "Ignorance is not an excuse". If indeed no one has informed them before, warn them to change it before the next round or they will receive a small <12 ban.
7. You see a Sulaco Doctor with an M41A rifle strapped to his back running around the briefing area. There are wounded in medbay, and the other doctor is in surgery.
Bwoink the player. Ask them to explain themselves. As per the rules, sulaco doctors are not allowed weapons stronger than a pistol unless the Sulaco is being boarded. Add a note to the players Ckey. If previous notes, apply a medical job ban for the relative amount of time based on the amount of medical rule breakings the player has done.
8. You see a Squad Medic performing surgery on the planet. He claims it was because there were no doctors in the FOB.
Bwoink the Medic. Inform them that Squad Medics are not allowed to perform major surgeries, and should only be stabilizing people to send them back to the Sulaco. If first offense, issue a warning and add a note the players Ckey. If the player in question has had a previous incident regarding this, job ban them from Squad Medic based on frequency of past incidents.
9. The round is stagnating. There are 30 marines and 4 aliens. The marines won't leave the Sulaco, and the aliens won't attack. What would you do, to "encourage" the sides to engage each other?
Wait for the mother report and queen mother report. If the neither side attacks,inform the marines that the USCM will consider the USS Sulaco and all of its crewmembers rogue if the mission is not continued. If this doesn't work, deploy a PMC team to shove the corporate fist into the marines and tell them to get down the surface.
10. The round ends, and a marine starts unloading his weapons on other marines.
Apply a note and a 3 hour ban for EORG.
11. A marine who has recently awoken is using soap to slip other marines.
Ignore it at first, if he causes a major disruption, inform the MP of his last known location through a mother report, and what he has done, along with adding a note if seriously disturbing.
12. A marine has killed another marine. When you ask him why, he said it was because the other marine had punched him.
Ask the player he killed his side of the story, then Determine if it was proper escalation. If so, inform the player he killed that he can kill the marine that killed him, but that marine did not break a rule as it was proper escalation. If the marine did not properly escalate it, apply a 12 hour ban, increasing in time with the amount of bad notes the player has for related incidents.
Any additional information you'd like to add?
I've been very critical of staff in the past. However, this does not make me a bad admin, in fact, I believe it will improve my stature and overall make me more self-critical and less likely to make mistakes. I'm also pretty good at german if that's relevant. I don't hold grudges against staff or players, and I try to base my decisions off of logic and reason over feeling.

the only nitpick I got is answer four, shoulda bwoinked the researcher and told him not to hand out napalm grenades to standards. Ive seen you ingame and on the forum some, so, I'm INCLINED to +1 this but, for now Ill stay neutral. Also, the minimum ban is 3 hours, not 12. so

Yeah, I should have included that. My bad. Also, I wasn't trying to say the minimum ban was 12 hours, I was just saying that for that action I would apply atleast a 12 hour, but I suppose that should be a little less harsh. So very few complaints on this one.
A few times in there I see you saying "maybe add a note" If you have to talk to a player about a rule related incident, always add a note. Always.
On the researcher question, you only failed to mention the liability of the researcher. Researchers are also partially responsible if a marine uses their grenades, but Cas covered most of this above.
As for the nuke codes, you failed to mention that only head staff can give out nuke codes. Unless you somehow usurp Feweh, like I plan to within the next few months, you will never be in a position where you can give out nuke codes.
Besides all that, I've seen you in game and you're not half bad. I'll go ahead and say +1.

Thanks. I will keep the adding notes policy in mind. Rereading my app, I had a lot of what you said in my head, it just didn't reach my app. As for the nuke codes, I did not know that, although that is probably just ignorance on my part.
