What other servers do you play on? I played a bunch on TG until I got on CM, haven't played any other server than CM for the past two weeks or so.

Are you now or have you been banned on any servers? Which ones? I got banned on yogstation for a long time ago, not banned now, however.

Your appeal, including evidence (screenshots, etc):This was a long time ago, but I believe it had something to do with having to leave. So me, being the new player that I was, shot themselves and left, instead of going to cryo. And that's all I remember. I don't have any evidence for this, but that's what happened in my memory, atleast.
