You Can Do Bone Surgery On Missing Limbs

Bug Description:

You can do bone surgery (and presumably other surgeries) as normal on missing limbs.

Steps to reproduce:
1. Commence limb replacement surgery on a missing limb.
2. Attempt to combine with fracture repair surgery.
3. Watch in amazement as the fracture repair surgery actually proceeds on the missing limb.
