Your appeal, including evidence (screenshots, etc). If you were in the wrong, explain what you did that was wrong, and describe how you have or will improve your behavior. To be honest, I can't remember ANYTHING related to my ban. The maximum I remember back on CM is me playing IO and marine for few rounds before leaving CM. I probably got my CID pinched when I was completely new to SS13 and simply clueless, thus the "evasion and 18plus content" ban. But now that I got interested in CM, i'd like to be unbanned.
